CONSENT

FOR THE PROCESSING OF PERSONAL DATA

I, as the subject of personal data, by providing this consent, voluntarily, in accordance with Articles 2, 6, 11 of the Law of Ukraine "On Personal Data Protection" dated 01.06.2010 No. 2297-VI (hereinafter referred to as the Law), provideGrid Labs Software Limited Liability Company(EDRPOU code 46270016, location: Ukraine, 04053, Kyiv, Kyyanivskyi Str., building 3-7, office 110 B, e-mail: office@gridlab.com.ua) (hereinafter referred to as the Company) consent to the processing of my personal data in accordance with the terms set out in this Agreement, as well as in the Terms of Confidentiality and Processing of Personal Data of the Company (hereinafter referred to as the Terms, the text of which is available for review on the official website of the Company).

The owner of personal data processed in accordance with this Agreement is Grid Labs Software Limited Liability Company (EDRPOU 46270016). The processing is carried out in the personal data database "GridLab EMS - Clients and Users".

1. Composition of personal data:surname and first name; position and name of employer; contact phone number and email address; login and authentication data in the software complex "GridLab EMS" (hereinafter referred to as the Software Complex); IP address, data about the device from which access is made, and logs of my activity in the Software Complex, including those obtained using cookies; information provided by me when contacting the Company, as well as other personal data specified in Section 4 of the Terms.

2. Purpose of processing:providing access to the Software Complex and authentication; execution of the agreement on the provision of services for access to the Software Complex; technical support; sending service messages; ensuring information security; compliance with the requirements of the legislation of Ukraine.

3. Processing and notification conditions.The terms of storage of personal data, measures to protect them, and the list of persons to whom such data may be transferred are determined by the Terms and Conditions, which I have read.

4. The company processes my personal data on the following grounds:

  • - consent of the Personal Data Subject;
  • - conclusion and execution of contracts;
  • - on other grounds provided for in Article 11 of the Law of Ukraine "On Personal Data Protection".

5. I, as the Subject of Personal Data, confirmmy consent to: the transfer of my personal data by the Company upon requests from state, investigative, and judicial authorities, the use of my personal data by the Company in accordance with the law and the Terms.

6. The consent I have given to the processing of my personal data does not requirenotifying me about the transfer of my personal data to third parties in accordance with the provisions of Article 21 of the Law of Ukraine "On Personal Data Protection".

7.   I, as the Subject of Personal Data, have the rightsubmit a reasoned request to the Company to object to the processing of their personal data, submit a request to change or destroy their personal data by any owner and manager of personal data, if this data is processed illegally or is inaccurate.

8. I, as the Subject of Personal Data, have the rightwithdraw this consent at any time by sending a message to the Company's email. The withdrawal of consent is not retroactive and does not apply to processing carried out on other legal grounds provided for in Article 11 of the Law. The withdrawal of consent makes it impossible to use my account and results in the termination of my access to the Software Complex.

By providing this consent, I confirm that I am informed about my rights as a personal data subject, as defined by the Law, about the composition and purpose of collecting my personal data and about the persons to whom they are transferred, as well as about the fact that my personal data is included in the personal data base "GridLab EMS - Clients and Users", the owner of which is the Company, and in this regard, I do not require further written information about the above circumstances.

USER CONFIRMATION

Confirmations are provided by ticking individual boxes in the Software Complex interface during the User's first login. Ticks 1 and 2 are mandatory, and access to the Software Complex functionality is not provided until they are ticked.

Checkmark 1 (required): "I have read the Terms of Confidentiality and Personal Data Processing."

Checkmark 2 (required): "I consent to the processing of my personal data under the terms set out in this Consent."

The fact of placing each mark is recorded by the Company, indicating the account identifier, date and time, IP address and revision number of the document that the User reviewed.
